Description:  For years, AHHC of NC has published the Partners In Quality Care newsletter for in-home aides. This popular publication includes an informative newsletter, post test, answer key, and certificate.  It is designed for one hour of educational credit that includes: 1. Review of Objectives; 2. Reading of educational material from the Partners in Care Newsletter; and, 3. Satisfactory completion of a test.  The monthly newsletter serves as an adjunct to satisfy your CMS requirement within the Medicare Conditions of Participation, and the newsletter topics are timely, relevant to home care, and can help enrich your in-home aides’ knowledge base.

Comments from KHCHA members opting to renew their subscription:

    • We find these newsletters very insightful and relevant for our home health aides and homemakers.
    • I’d like to renew - this format worked beautifully to keep our HHA up to speed, and our JCAHO surveyor was very happy with the documentation of the training.  Thanks!

    Description

Rehospitalizations continue to be a focus of health care quality strategy programs, including Home Health Value Based Purchasing. The home health care industry serves patients with many risk factors for rehospitalization, including the aged population and those with multiple medical and social comorbidities. This five-part series will focus on how reduction of rehospitalizations in the home health space is monitored and incentivized through quality and payment programs and will include management strategies for some of the most common comorbid conditions in our patient population: Heart Failure (HF), Chronic Obstructive Pulmonary Disease (COPD), Diabetes Mellitus (DM) and Mental Health Crises.

    Objectives

    • Recognize areas of policy and practice to be assessed and/or modified that will lead to better quality of care as it relates to chronic, comorbid condition management and rehospitalization rates for the agency's population served.

    • Demonstrate an understanding of Home Health Value Based Purchasing Measures and how they are impacted by comorbid conditions.

    • Distinguish status quo interventions versus best practices for chronic disease management, leading to better patient outcomes, decreased rehospitalization rates, and proper payment for care in the current Home Health Care environment.

    • Identify areas of improvement, both personally and agency-wide, where best practices can be implemented to reduce cost, increase coordination and communication in a patient-centered care model, and improve quality of care and quality of life for the home health patients they serve.
