The Kansas Home Care & Hospice Association is pleased to bring you this educational program through an agreement with the Association for Home & Hospice Care of North Carolina…

Description:  For years, AHHC of NC has published the Partners In Quality Care newsletter for in-home aides. This popular publication includes an informative newsletter, post-test, answer key and certificate. It is designed for one hour of educational credit that includes: 1. Review of Objectives; 2. Reading of educational material from the Partners in Care Newsletter; and, 3. Satisfactory completion of a test. The monthly newsletter serves as an adjunct to satisfy your CMS requirement within the Medicare Conditions of Participation, and the newsletter topics are timely, relevant to home care and can help enrich your in-home aides’ knowledge base.

    Feedback from KHCHA subscribers:

    • We find these newsletters very insightful and relevant for our home health aides and homemakers.

    • I'd like to renew - this format worked beautifully to keep our HHA up to speed, and our JCAHO surveyor was very happy with the documentation of the training. Thanks!

    • Just want to let you know that our employees love this format of education. They even comment on how informative the materials are and how much they enjoy the topics. We really appreciate that KHCHA provides this program at such a reasonable cost.

    • This training series was well received by our last JCAHO surveyor--it definitely serves the purpose very well!

    Audio-Conference Description

Home care nurses, including those providing hospice and palliative care and private duty services, are the most vulnerable to professional liability claims of all nursing specialties. Recent examples of malpractice cases focus on failure to communicate pertinent health information with other providers and/or patient/family members, failure to monitor patients, and failure to document or falsifying documentation. What are some recent examples of malpractice cases filed against providers of services to patients in their homes? What concrete steps can post-acute providers take to avoid malpractice claims? What role does complete, accurate documentation play in many malpractice cases? How can providers achieve it? Join us to help ensure that your company is not at the top of the malpractice “hit list.”

    Getting Ready for the
    Hospice HOPE Assessment

    presented by

Katherine Morrison, MSN, RN, CHPN, CCS-HH
    Director Advisory Services
    WellSky

    Wednesday, May 14, 2025
    9:00 to 11:00 a.m. Central Time

    (Recording available through June 13)

The HOPE standardized assessment will be hitting hospice providers in the near future.  Will you and your team be ready for this major change?  The HOPE tool will be designed to collect specific information about the patient’s hospice experience by capturing patient and family care needs in real-time and throughout the hospice stay with the flexibility to accommodate patients with varying clinical needs. CMS has deployed standardized tools for other provider types and hospice can learn from these tools to prepare their organization and team for utilization of HOPE.

    Objectives

    • Discuss the HOPE assessment to date and CMS plans for utilization.
    • Compare and contrast Hospice Item Set (HIS) and HOPE.
    • Describe components of CMS standardized tools for other provider types.
    • Identify target areas for hospice provider focus in preparation for HOPE deployment, including inclusion of HOPE in comprehensive assessment and care planning.

    Included materials:

Hospice Outcomes and Patient Evaluation Tool: Leaders need to do more than HOPE Participant Guide

    ****12:00 p.m. to 1:30 p.m. Central Time****

    Audio-Conference Description

Home health, hospice, private duty and HME providers are routinely subject to audits of all kinds, including by MACs, SMRCs, RACs and UPICS. The purpose of this teleconference is to help providers respond successfully to claims denials of all types. What is the first step providers should take to respond to audits? What should providers do if they find deficiencies in documentation? Can providers supplement and/or amend records before submission? If so, how should providers do so? What about summaries of records? Are they useful and if so, how? What should be included in summaries? Providers continue to take a beating with regard to audits so now is the time to understand the best way to beat them. There will be plenty of time for questions during the teleconference and extensive handouts will be provided.
