Don't Get Lost on the Way: Creating a Referral Process That Leads Somewhere


Description

The front-end referral inquiry to start of business (admissions in health care) is the life blood for any organization, health care or not!  Learn in this session what the anatomy of a referral inquiry to admission process is for hospice and home care.  The five key elements that will be explored in this session are:

1.  Knowing you are capturing ALL referral inquiries.
2.  Creating a strong intense sense of urgency
3.  A well-developed eligibility process
4.  A 90-day pending list and work process.
5.  Organizational commitment and wisdom

    Kurt A. Kazanowski, MS, RN, CHE
    Vice President of Hospice and Palliative Care, Moreslife, West Palm Beach, FL                                                                      

    Kurt is a native Detroiter and now lives and works in West Palm Beach, Florida. His current position is that of Vice President of Hospice and Palliative Care at Moreslife in West Palm Beach. He is an accomplished health care executive in the areas of hospice, home care and post-acute care services. He has served in executive roles for the following organizations, Hospice of Michigan, Seasons Hospice, VITAS and Trinity Health Services to name a few. He also currently is the owner of Caregiving Experts, a private care home care company in Plymouth, Michigan. For 10 years prior to Morselife, Kurt was the Managing Partner for Hospice Advisors, a boutique hospice and home care consulting firm. Kurt also is an author, speaker and health care entrepreneur. His recent books, “A Son’s Journey: Taking Care of Mom and Dad” and “The Seven Pillars of Growth” provide a platform for his professional speaking. Kurt holds a BSN from Mercy College; a master’s in health service administration from Wayne State University and is a member of the National Speakers Association.

    Continuing Education

    Kansas Home Care & Hospice Association is approved as a provider of CNE by the Kansas State Board of Nursing. This course offering is approved for 1.5 contact hours applicable for APRN, RN, or LPN relicensure. Kansas State Board of Nursing provider number: LT0287-0314. 


    Other disciplines will need to apply independently – a certificate of attendance for 1.25 CE hours will be provided.
